Let's begin by opening a new image 500 x 500.  I know this is very large, but I
like a lot of room to work.  We can of course, crop after we are all finished.

Open your colors image on your desktop, as well as the bear and the rattle images.

Make a new layer and name it "body".  Load the "B_Body" selection from disk.  Flood fill with the dark brown, then go to selections, modify, contract by 1 and flood fill with the skin color.  Deselect, then go to selections, select all, float.  Add the following cutout twice, changing the second cutout to -2 Vertical and Horizontal.  Use the dark brown for the shadow color.  Then you may deselect.
Add a new layer and call it "diaper".  Load the "B_Diaper" selection from disk.  Flood fill with  white only once.
Deselect and add the following drop shadow twice changing the second drop shadow to -2 Vertical and Horizontal.
Add a new layer, then take your selection tool set to circle, feather 0, antialias checked and draw a small circle in the middle of the ties on the diaper to make a knot.  Keeping selected, flood fill with white and then add the following inner bevel. 
Keeping selected add the drop shadows as above.  Then you may deselect, copy and then paste a new selection moving it into place on the other side of the diaper between the ties.  At this point turn off all layers except the diaper and knot layers and merge visible and rename diaper.

Add a new layer and name it "head".  Load "B_head" selection from disk and flood fill as you did for the body.  Do the two cutouts as you did for the body, changing the second cutout to -2 Vertical and Horizontal.  Deselect.
Make sure that the head layer is on top of the body layer.

Add a new layer and call it "arms".  Load the "B_Arms" selection disk and flood fill with the dark brown and skin color as above and do the two cutouts as above.  Move this layer below the body layer.  Deselect.  Make sure you are saving!

Next open up the bear image, copy it and then paste it as a new layer, renaming the layer "bear".  Make sure that this layer and the rattle layer are between the body and arms layer. Open up the rattle image, copy it and paste it as a new layer, renaming it "rattle".  Refer to the picture at top of page for placement.
