REQUIREMENTS FOR PASSPORT ISSUANCE

I. RENEWAL

1. Old Passport

2. 3 pictures, size 1.77” x 1.37” with white background

3. Proof of any following documents showing full middle name in case the passport is the old format:

a. Birth Certificate

b. Baptismal Certificate

c. Voters Identification

e. School Identification, Diploma or Form 137

f. Driving License

g. Marriage Contract

h. Philippine Public Documents, i.e. NBI, Land Titles

If the above documents are not available or cannot be presented, an affidavit can be executed from the Embassy attesting the full middle name of applicant in lieu of the above documents. The validity of the passport, however, is only for one year until showing of the above required documents. Further, allowing the execution of affidavit is discretionary to the signing officer.

II. LOST PASSPORT

1. Photocopy of the lost Passport, if not available secure an Authenticated Birth Certificate from Manila

2. 3 pictures, size 1.77” x 1.37” with white background

3. Police Report ( with English translation)

4. Affidavit of Lost Passport (to be secured from the Embassy)

5. Proof showing full middle name such as: (Old Format)

a. Birth Certificate

b. Baptismal Certificate

c. Voters Identification

e. School Identification, Diploma or Form 137

f. Driving License

g. Marriage Contract

h. Public Documents like NBI, Land titles

If the above documents are not available or cannot be presented, an affidavit can be executed from the Embassy attesting the full middle name in lieu of the required documents. 6. Confirmation from DFA of the issuance of passport (meaning the applicant has to wait at least 15 working days before the issuance)

C. NEW PASSPORT (Issued for the First time - children born in Jordan)

1. Registration of the child’s birth to the Embassy

One. Report of Birth (4 copies) . Required to submit with the ff:

1. Copy of Passport of Parents

2. Hospital Certificate or Physician Certificate or Medical Certificate or Birth Certificate issued by the Jordanian Civil Registry with English translation (if not available testimony of person having the knowledge of Birth under oath or notarized )

3. Marriage Contract of Parents (if not available an Affidavit of Illegitimacy to be executed before the Embassy).

4. If the child is legitimate, a certification from Ministry of Interior with English translation.

2. 3 pictures, size 1.77” x 1.37” with white background

D. For Fake (Assumed name/data) Passport

1. Surrender of the fake passport to the Embassy

2. Authenticated Birth Certificate, and if married Authenticated Marriage Contract

3. Affidavit of Explanation which can be secured from the Embassy

4. 3 pictures, size 1.77” x 1.37” with white background

5. Registration with the Embassy
