Form Name	Purpose
Activity Permission	Activity attendance and medical release form. This form is needed prior to the scout attending any troop activity other than meetings.
Class 1& 2 Physical	All scouts participating in any troop activity. Can also use class 3.
Class 3 Physical	All adults and scouts participating in High Adventure Programs.
Vehicle Insurance	Information needed by council for tour permits.
Internet Permission	Provide permission and method of listing scouts name on the web.
Tour Permit	Must be filed prior to any trip (can be faxed to 325-2122).
Money Earning Permit	Must be approved prior to money earing activity (can be faxed to 325-2122).
Advancement Report	Used for reporting advancements to Council Office.
Merit Badge Councelor	Merit Badge Counselor registration form.
