The chords go like this: Bb C F C That should be enough to put it all together. The adventurous are invited to use these chords to play it on the piano. I figured it out be ear on the guitar, because it's easier for me to hear and identify chord patterns that way, but playing it on the piano gave me a much better sense of what to do with each chord.
