Feline Healthcare Partnership:

As a partner in your practice, Fort Dodge Animal Health is committed to the well-being of your feline patients. We realize, as you do, that this commitment begins with a solid foundation in disease prevention.  Vaccination of healthy animals plays a very important role in your prevention program.

As your healthcare partner, we also realize that certain issues can often times complicate decisions to promote and/or accept healthcare programs. One such issue is feline injection site-associated sarcomas. Experts agree that sarcomas are naturally occurring phenomenon that can be associated with any injectable that is administered to a genetically predisposed feline. The experts also agree there is still much research to be done to determine specific causative agents of sarcoma development. Yet, vaccination has been implicated as one potential factor and as a result the most fundamental approach to a disease prevention program has come under extreme scrutiny.

As your healthcare partner, it is our goal to provide you with the finest quality feline vaccines available. FDAH wants you to feel confident when administering our feline vaccines.  To ensure this confidence, we will provide you assistance when assessing injection site-associated masses in your patients vaccinated with our products.

With this in mind, we are proud to announce the "Feline Healthcare Partnership".

Assuming the provisions outlined below are met, Fort Dodge Animal Health will reimburse the costs associated with the treatment of an injection site-associated sarcoma as follows:

* Fort Dodge Animal Health will reimburse the veterinarian for services up to $1,500 for treatment costs associated with treating an injection site-associated sarcoma.
* Should the veterinarian prefer, Fort Dodge Animal Health will donate the amount they would have been reimbursed to the "Feline Fibrosarcoma Task Force" (VAFSTF) on their behalf.

                Provisions for this Partnership

*This program is in effect for all cats vaccinated with Fort Dodge Animal Health feline vaccines pursuant to the terms and conditions as of August 1st 2000 and may be terminated at any time at the sole discretion of Fort Dodge Animal Health.
*Use of any brand of feline rhinotracheitis, panleukopenia, calicivirus, chlamydia, feline leukemia or rabies vaccines, after 8/1/00 at the affected injection site other than Fort Dodge Animal Health vaccines will void this agreement.
*The time frame between vaccination and occurrence of the injection-associated sarcoma must not exceed 2 years.
*Cats must be vaccinated by a licensed veterinarian with an established client-patient relationship.
*Diagnosis of the injection site-associated sarcoma must be confirmed by histopathologic examination.
*Documentation of the vaccination records must be maintained on file by the veterinarian and be available for review by Fort Dodge Animal Health.
Vaccine records must include vaccine brand, serial number and location of injection.

*The payment obligation of Fort Dodge Animal Health under this program shall be solely limited to $1,500 for treatment costs as specified above.
Any and all other costs related to or associated with an injection site-associated sarcoma following use of product are expressly excluded.
*This program does not cover cases currently under review.
