My ParenTime's Printable Babysitter Checklist (This is a great form that you print out and then fill in with important info that your sitter should have:  emergency numbers, doctor’s name, allergies, etc.  I've seen tablets of these forms in stationary and children's shops, but now you can print your own for free!)
