    First off, look for a heel of layered wood and/or leather--not the solid block plastic.
   
Height should be measured in strait vertical line--not along slope of rear of heel.  Get that established right away if you're orderin' up a custom pair.  Be aware that most makers don't much like makin' a heel over 2"--and if that's your preference & they hesititate to make it that high you oughtta go elsewhere.  A 2" or higher heel calls for a high, firm arch, and the biggest mistake I see is makers tryin' to fit a high heel on a shank that's made for a lower heel.  It's not gonna be comfortable (you'll feel like you're walkin' on a slope, with your weight pushin' down on your toes)--and it won't look right.  In the boot pictured above the heel & shank combine as an ideal whole:  the heel looks in proportion--not "tacked on."
