The undersigned adopter (s) do hereby release, discharge and hold
harmless NEW ENGLAND SHELTIE RESCUE and/or any other person, firm, or corporation charged
or chargeable with liability, their heirs, administrators, executors successors and/or
assigns from any and all claims, damages, costs, expenses, loss of services, actions and
causes of actions belonging to said adopter (s) arising out of any act or occurrence from
the present time, particularly on the account of the adoption of the above named dog.
CONDITIONS OF ADOPTION
- The above said dog is free of any liens or encumbrances except for
the conditions specified in this contract.
- Adopter agrees to pay the sum of $____________ for the adoption of
said dog.
- Adopter agrees to have said dog SPAYED/NEUTERED within two weeks
from the date of adoption or a $1000.00 penalty will be due to New England Sheltie Rescue.
The adopter will forward the spay/neuter certificate to New England Sheltie Rescue.
- It is agreed that said dog will be maintained within a fenced-in
yard or an approved dog kennel. During times of inclimate weather said dog will be housed
indoors away from the elements. The above mentioned dog will AT ALL TIMES be under the
ADOPTERS control and is never to be allowed to roam free.
- It is understood that New England Sheltie Rescue assumes NO
responsibilities for damage , or injuries caused by the above mentioned dog.
- This dog is on a two week trial period any time within this two
weeks the dog may be returned for a full refund , if after this two week trial the dog
does not work out the dog still MUST be returned to New England Sheltie Rescue but a
refund will not be given.
- The adopter agrees to keep New England Sheltie Rescue informed of
any address changes. It is also understood that at NO TIME will the above mentioned dog be
SOLD , GIVEN AWAY , ABANDONED , SOLD TO SCIENCE FOR RESEARCH , USED FOR ANIMAL SACRIFICES
, WILL NEVER BE USED AS FOOD FOR HUMAN CONSUMPTION OR THAT OF OTHER ANIMALS , WILL NEVER
BE GIVEN TO OR USED AS OR IN THE TRAINING OF FIGHTING ANIMALS.
- It is also understood that the adopter will keep said dog up to
date on all shots , check for heart worm every year , and a visit to a veterinarian at
least once per year. The adopter is responsible for any and all vet bills incurred for the
life of this contract.
- The adopter will inform New England Sheltie Rescue of the
dogs death and also the cause of death.
- Changes , additions , or any other alterations of this contract MUST
BE IN WRITING and signed and agreed upon by ALL PARTIES INVOLVED.
- Special clauses :
- Failure to honor any part of this contract will result in a
penalty of not less than $1000.00 per occurrence payable to New England Sheltie Rescue.
ANY AND ALL LEGAL FEES AND COURT COSTS NECESSARY TO ENFORCE THIS CONTRACT WILL BE THE
OBLIGATION OF THE ADOPTER INCLUDING ALL THOSE INCURRED BY NEW ENGLAND SHELTIE RESCUE.
- New England Sheltie Rescue gives NO warranties covering the health
or temperament of the above mentioned dog both expressed and implied .
- THE ENTIRE AGREEMENT BETWEEN THE PARTIES ARE CONTAINED HEREIN
- Your signature below means that you (the adopter) are in full
agreement with the terms of this contract.

IN CONSIDERATION OF MY RECEIVING THE SHETLAND
SHEEPDOG DESCRIBED BELOW, I HEREBY AGREE TO CARE FOR THIS DOG HUMANELY, including
adequate food, shelter, water and veterinary care, and to license this dog in accordance
with the laws in the jurisdiction in which I reside. I also agree to keep the dog as a
house pet, to be exercised in a fenced yard or on a leash and NOT to be free to
Roam the Streets. I also agree NOT to sell, trade, or dispose of this dog in any
way, but to RETURN the dog to Southland Sheltie Rescue if at anytime I wish to relinquish
custody. I understand that I have two weeks from this date to return said dog for a full
refund, minus any vet bills. I further agree that the dog shall not be used for MEDICAL or
any other EXPERIMENTAL purpose, shall not be chained or tied, and shall not be used for
breeding. It is understood that Southland Sheltie Rescue may examine and may make inquiry
about said dog at any time. If not satisfied with the condition of the dog or the
conditions in which it is being kept, said dog may be reclaimed by Southland Sheltie
Rescue immediately. No AKC REGISTRATION CERTIFICATE IS APPLICABLE IN THIS
TRANSACTION. Southland Sheltie Rescue Inc. cannot be held responsible for the conduct
of this dog after its adoption. All interaction between this dog and any minor child or
children is the responsibility of the owner (s) of this dog.

Adoption Contract
This agreement is made and hereby entered into, on this _______ day of
__________________________, by and between Central Illinois Sheltie Rescue, and
___________________________________, hereafter referred to as Adopter. The dog described
below will hereafter be referred to as "Rescue sheltie", "Dog",
or "The dog", or "This dog". The purpose of this contract
is to ensure that adopter provides this dog with the best possible care and treatment
throughout its life.

In consideration of a donation of $ ________ ; Central Illinois Sheltie Rescue agrees
to allow Adopter to adopt, a Rescue Sheltie known as _____________________, under the
terms set forth in this contract.
Central Illinois Sheltie Rescue warrants that at this time and to the best of our
knowledge this dog:
_____ Is current on all shots, is spayed or neutered, is heartworm negative, has been
wormed, has been
groomed, and is taking flea and
heartworm prevention medication.
_____ Is in good health, and free of any diseases at this time.

Adopter understands and agrees to abide by the following terms of adoption:
- If, within two weeks, for any reason, you decide this dog is not right for you or your
family, you may return the dog to Central Illinois Sheltie Rescue for a refund of your
donation.
- At this date of adoption, Adopter agrees to assume all medical expenses, food, grooming,
and any other costs associated with this dog. If this dog needs veterinary care within 2
weeks of it's adoption the adopter agrees to either pay for the vet care himself, or
return the dog to Central Illinois Sheltie Rescue for a refund of the adoption donation.
Central Illinois Sheltie Rescue does not pay for, nor reimburse for any veterinary
services performed for any dog that we do not have physical custody of.
- The Adopter agrees that ownership of this dog is not transferable
. The Adopter
agrees that if for any reason he/she is unable to continue to house and care for the dog,
or if he/she should decide that he no longer wants to keep this dog, that he will
immediately return the dog to Central Illinois Sheltie Rescue. In the
case of a long distance adoption, it is the adopter's responsibility to transport the dog
back to Central Illinois Sheltie Rescue in a safe and approved manner. There will be
no refund after the 2 week period, unless approved by a CISR representative.
- Adopter agrees to treat this animal in a humane and loving way at all times, and to make
sure that anyone temporarily caring for this dog will do the same.
- The Adopter agrees to provide this dog, and all other animals he/she owns with proper
veterinary care as is recommended by his or her veterinarian, for the rest of each
animals life. This includes yearly vaccinations and prevention of parasites, and
fleas and ticks. Adopter also agrees to keep this dog on a monthly heartworm prevention
and flea control program throughout the dogs entire lifetime. We
recommend Sentinel or Interceptor. Adopter further agrees to take this dog to
his/her veterinarian for a routine check up at least once every year, and to follow the
advice of his veterinarian. Adopter also agrees to provide any other medical
attention the dog may need in its lifetime. Adopter further gives Central Illinois
Sheltie Rescue permission to verify this care by contacting and talking with
adopters veterinarian.
- Adopter understands that Shelties are a very shy breed, and are a great
"runaway" risk during the first 30 days in a new home. Adopter understands to
take extra precaution during the first month to prevent this dog from getting loose
outside, or running out of a door that was left open. Adopter fully agrees
that IF this dog were to get loose, and become lost, adopter will contact Sheltie
Rescue immediately. Adopter agrees to follow the search instructions provided by
Sheltie Rescue exactly as provided. Adopter agrees to provide any and all resources
needed (both financial and/or otherwise) in the search and recovery of the dog.
- Adopter agrees to have an ID tag made, and attach it to the dogs collar. Adopter
further agrees to make certain the dog wears its collar with ID tag at all times.
The information on the tag should include your at least your phone number. Adopter
agrees to make certain this ID tag always has current information on it .
- Adopter agrees to license this dog according to any local ordinances.
- Adopter agrees to keep this dog clean and groomed at all times.
- Adopter agrees to feed a wholesome, premium dog food, and have fresh water available at
all times. Generic dog food, store brand dog foods, or no-name dog foods are not
considered premium food.
- This dog is being placed with the strict understanding that it is going to a private
home, to live as an indoor pet. The Adopter agrees that this dog will reside in the
home of the Adopter, not outside. This means that this dog will be inside
adopters home except for potty breaks and supervised playtime outside.
- Adopter agrees that this dog will never be allowed off a leash when it is outdoors
unless it is in a fenced-in area or fenced yard. This dog may never be left
outdoors if you are not home.
- Adopter understands that Central Illinois Sheltie Rescue will never place a known biter.
However, Adopter is fully aware and does understand that any dog may bite if provoked, or
if sick or injured. Therefore, Adopter agrees to use due caution when allowing any
stranger or child to be near this dog, and will never allow any child to threaten,
squeeze, poke, step on, or otherwise pose a perceived threat to this dog.
- Adopter understands that no one can predict what medical or temperament problems a dog
may have in its lifetime. No one from our organization may guarantee anything other than
the current health and temperament of this dog.
- The Adopter agrees that he/she is not acting as, or in part of any agent in the purchase
of this Dog, and that the Adopter will not sell this Dog to any other person, agent, or
pet store.
- Adopter grants permission for a representative of CISR to contact adopter's veterinarian
to confirm the care given to this dog, and to visit the home at any time to check on the
wellbeing of this dog. Adopter further agrees that Central Illinois Sheltie Rescue
may repossess this dog upon any evidence of neglect and/or abuse, or any breach of any
part of this agreement, at no expense to the Central Illinois Sheltie Rescue, and with no
refund of any monies. IF Central Illinois Sheltie Rescue does ask for this dog to be
returned to them, Adopter agrees to do so within 24 hours.
- Should Central Illinois Sheltie Rescue need to seek legal action against the Adopter for
any violations of this Agreement, the Adopter hereby agrees it will assume any and all
attorney costs, court fees, and any other legal fees incurred, in the process of
enforcing this contract, or repossessing the dog.

harmless Long Island Shetland Sheepdog Fanciers Rescue and anyone else associated with the
LISSF Rescue from any charges or claims arising from the adoption of:

THE UNDERSIGNED agrees to abide by the "Terms of
Adoption" listed below:
- Return
: The adopted dog may be returned to LISSF Rescue within
two weeks of the date of adoption for full refund of donation if the adopter is not
satisfied with any aspect of the adoption. After two weeks from the date of adoption no
refund will be given if the dog is returned to LISSF Rescue.
- Transfer of ownership
: The adopted dog may not be transferred
to any other person for any reason. If the undersigned adopter is forced to relinquish
custody of the dog at any time, said dog will be given only to LISSF Rescue.
- Refund
: There will be no refund or reimbursement for any
expenses incurred by the adopter for the adopted dog, even if it is returned to LISSF
Rescue.
- Health Program
: The dog will be licensed in
accordance with the laws in the jurisdiction in which the dog resides. The adopted dog
must wear an identification tag at all times. The adopted dog will be cared for humanely,
including good quality food, shelter and water available 24 hrs. a day. The dog is to be
seen by a veterinarian yearly, given all necessary vaccines, checked for heartworm each
spring, stool sample checked for worms every six months and seen by the vet when ill. THE DOG IS TO BE EXERCISED IN A FENCED YARD OR ON A LEASH AT ALL
TIMES AND NOT FREE TO ROAM.
- Reason for adoption
: The adopted dog is to be kept as a house
pet; it shall not be used for medical or any other experimental purpose; it shall not be
chained or tied, and it shall not be used for breeding.
- Loss
: If the adopted dog is lost or stolen, the adopter will
notify LISSF Rescue immediately, at least within (3) days, so LISSF rescue can help locate
the adopted dog.
- Reservation of Rights
: Long Island Shetland Sheepdog Fanciers
rescue reserves the right to enforce this contract in order to protect the welfare of the
adopted dog. It is understood that LISSF Rescue may examine and will make inquiry about
said dog at any time. If the terms and conditions of the adoption Agreement are not
upheld, LISSF Rescue reserves the right to terminate the Agreement and to reclaim the dog.
- Return Check Charge
: There will be a $25.00 charge on all
returned checks submitted by the adopter.

I hereby acknowledge receipt from Long Island Shetland Sheepdog
Fanciers (LISSF) Rescue of the dog described above. I accept the dog as is and assume all
risk and financial responsibilities of ownership. I hereby release LISSF Rescue from any
liability associated with any illness of the dog or damage; accident or injury hereafter
caused by said dog. I agree that LISSF Rescue will not in any way be held responsible for
the conduct of this dog after its adoption. I understand that some or all of the
information provided on the Adoption Agreement regarding said dog could have been received
by LISSF Rescue from a third party; I understand that LISSF Rescue does not warrant the
accuracy of such information. In addition, if the dog was found wandering or lost, I
acknowledge that LISSF Rescue has no knowledge of the prior owner(s) of the dog, and that
the prior owner(s) may contact me at any time with regard to retaking the dog. If this
occurs, the only remedy I will have is that LISSF Rescue will provide me with another dog,
if and when one is available and I so desire, at no additional cost to me. If at any time
I desire to relinquish custody, or LISSF Rescue demands its release for failure to comply
with agreement, I agree to return said dog to LISSF Rescue, making no charges of any
nature for licensing, care, food or other services or items. I agree to pay any reasonable
attorney fees and court expenses if LISSF Rescue must bring legal action against me to
enforce this Adoption Agreement. I understand that any sum of money I have given to LISSF
Rescue is to be used for its work in caring for this and other shelties and is not
refundable except as provided in "Terms of Adoption".
I hereby acknowledge that I have read and understood this
Adoption Agreement and agree to abide by all of its rules and regulations set forth
herein. I pledge that I will keep the dog described herein as a part of my family.
