Alison would be pregnant and wondering whose baby it was. She would eventually find out that there was a flaw in the wish and it was Rafe's. Lucy would go back to Kevin and Ian would disappear. Casey would come back down for good.

Livvie would have left Caleb and went crying to Jack and they would have gotten back together. Imani would have gotten killed. Casey came back from heaven for good to be with Ricky. Frank, Amanda, Victor, & Mary would come back and let us know where they been, since they are all vampires except Mary. Rafe and Alison find out that she is pregnant and she wants to tell him the truth but doesn't. Caleb knows Livvie will try to kill Alison and their baby so he wishes on his ring that she won't so she can't do it. Chris finds a vampire cure for Ian and Jamal. Ian and Lucy get married. Chris and Elizabeth hook up. Caleb sits back and waits for what is to come. Kevin has a complete recovery. Caz is shown more. Reese and Alison find out their sisters. Alison has her baby and it comes out with dark hair and a dark complexion and looks just like Caleb so then she is forced to tell Rafe. Rafe is enraged and tries to kill Caleb and a whole new war starts again. Caleb kidnaps Alison because he will not let a slayer raise his child.

Despite Imani being a werewolf she and Jamal stay together. Alison and Rafe find out they are pregnant, but Alison doen't tell Rafe until she's too far along that it's possibly Calebs. Livvie goes mad at the thought of Alison having her husbands baby. Ian and Lucy get married, Kevin heals and continues his work. Chris and Elizabeth get together but then everyone finds out that they are George not just him. Frank is visited by Karen, but he can't take it and tries to attack someone but they stake him and he dies. Ricky and Reese get together but not before she and Alison find out they are sisters. Then Casey comes back for good, and Ricky is torn.

Lucy would be torn between Kevin and Kevin would be visited by Karen (even though she is gone it is PC, I would have her back as an angel like Casey) and Kevin would long to be with her the same way Ricky longed to be with Casey. I would add a 3rd angel to the line up, Eve. Ian would think he is imagining Eve's spirit, and she will be trying to warn him of danger a head. Meanwhile Imani and Jamal will develop a relationship, and fight to be together even though she is a werewolf. Alison would have to deal with being pregnant with Calebs baby (but not know it yet) she would be plaged with flash backs of her night with him, as would he. They would have another night of passion but this time someone would find out... who would it be?? More forbidden circumstance between Ricky, Casey and Reese, and what would happen when Marissa comes back, would that mean bye bye for Casey for good?

The arc would focus on true love in the couples of PC. I'd reunited Kevin and Lucy and Eve would somehow come back to life and reunite with Ian and Danny. Everyone, including Caleb, would have become human again. Caleb and Livvie would marry and have a child of their own. There's been too much hate and supernatural focus in the last few arcs. I'm tired of the fight between Caleb and Rafe. The show should have lightened up some.

Eve would return for Chris, Ian and Lucy would run away together. Rafe and Alison would marry and Imani and Jamal would find love and Frank and Livvie would hook up again.
Endure

Rafe & Alison and Chris & Elizabeth would be the main characters in the arc. Rafe would find out about Alison's baby by running into Caleb in the park and then he would ask Alison about it and she would tell him. It would be a little rocky for awhile, with Rafe being upset about it. At the end of the arc, they would finally figure out that it was actually Rafe's baby in the first place and Rafe would forgive Alison because it wasn't her fault to begin with. Chris and Elizabeth would finally admit their true feelings for each other and then they would hook up. Wild and zany things ensue :) Since Livvie already knew about Alison's baby, she would leave Caleb and Caleb would have no problem with it. She would stay at Jack's, and then they would hook up for a short while, but during the end, Livvie would realize that she was wrong about Caleb and that she shouldn't have ended it with him because of that wish that was her fault, not his. So she would go back to him and they would get back together for eternity. Ian and Lucy would break up and Lucy would go back to Kevin.Eve would come back and tells Ian the secret cure to get un-vamped. They get back together and live happily ever after. Chris would find out that Eve is back and would introduce her to Elizabeth, and they surprisingly get along. They all become good friends and have often martini parties at Chris's place. Casey comes back to earth tells Ricky she will follow him wherever his career takes him and they will leave Port Charles together to be happy.

I think that there would have been the whole who's the Daddy with Alison's baby and it would end up being Rafe's. Rafe would swear revenge on Caleb after he found out that he slept with Alison. Livvie would have run from Caleb and left town being devastated. Caleb would try to find her..but Jack would find her and they'd get back together for awhile until Caleb found Livvie and he would try to get her back. After Alison found out about being pregnant Caleb would taunt her and hold the night they were together over her head until she finally told Rafe. Lucy and Ian would be stuffed down our throats some more. Jamal would swear to stand by Imani and that her being a werewolf didn't change anything....

I think there are too many things that COULD happen to make just a single guess. In just 1 episode we find out Jamal's girlfriend was definitely a werewolf, Alison was pregnant, and Lucy basically want's Ian to do whatever he needs to do to have a better life... All that would lead to a VERY interesting next season.
Oh Baby
Rafe & Ali plan for the baby not thinking that the baby could belong to Caleb (Since vamps aren't able to have children and they don't know about the wish). Then Caleb starts becoming more protective and interested in Ali. She confronts him and he tells her smuggly that it's his baby and not Rafe's and he tells her about the rest of the wish and Ali freaks out. Livvie wants to take Ali's baby from her and even kidnaps her but in the end through divine intervention (Maybe Ed), the baby turns out to be Rafe's all along. The wish Livvie made was something Heaven and Hell couldn't grant.
NEW-Illusion
The barn thing would have never happened. It would have been a dream Caleb implanted in Alison's mind as revenge. The baby would be Rafe's. Caleb and Livvie would have their own baby.
