Supplies here: the template is from Ann;
the man tube was made by SilNic,
the flower came from the internet.
 

Filters: Simple.

Foreground color  #424D66
Background color #ADB2BC
 

  1. Open the template in PSP.
    Go to File --> Export  --> Custom Brush as demonstrated below:
                        

    Fill in a name for the brush in the pop-up window.
     

  2. Open the flower tube.
     

  3. Add borders:
    1 px foreground color;
    1 px background color;
    1 px foreground color; repeat 1x;
    1 px background color.
    Sharpen 1x.
     

  4. Add border: 'Symmetric' should not be checked:
    Left  - 10 px;
    Top and bottom - 75 px;
    Right - 175 px - foreground color.
     

  5. Select the wide border with your Magic Wand.
     

  6. Effects --> Texture Effects --> Blinds. Appply with these settings:

                       
     

  7. Effects --> Edge Effects --> Enhance More.
     

  8. Effects --> Filter Simple: Pizza Slice Mirror.
     

  9. Deselect.
     

  10. Add a new raster layer. Find the brush that you have just made.
    Right-click 1x with your mouse on the canvas.
    Sharpen 1x.
     

  11. Move the layer so that the text is situated at the right.
     

  12. Use your Eraser tool to erase the parts that falls outside the text. Only the text should remain.
     

  13. Merge layers.
     

  14. Open the man tube. Copy and paste onto the canvas and drag it to the top left corner of the canvas. Reduce the opacity of this layer to 50% in your Layers palette.
     

  15. Merge all visible layers.
     

  16. Add borders:
    1 px foreground color;
    1 px background color;
    1 px foreground color; repeat 1x;
    1 px background color.
     

Your tag is finished.
You need 2 additional background images if you
want to use the script.
