Q. What is " HealthPlan USA"?

A.HealthPlanUSA is a
dental referral organization that offers memberships in its discount dental plan. Members can save 25% to 65% on all restorative and cosmetic work
(fillings, crowns, braces, etc.) and up to 80% on preventative work (teeth cleaning, x-rays, etc.) General dentristy and all specialties where available, are covered.

Since the dental plan is not insurance or a health care organization, all pre-existing conditions are covered except orthodontic treatment in progress. There is NO waiting period before you can start using the plan. There are NO deductibles, NO claim forms to fill out, and NO limits on visits to your dentist.

Q. How do I pay my Membership Fee?

A. Monthly or quarterly payments
are made ONLY by Electronic Bank Draft system, or by credit card. INVOICING IS DONE ONLY FOR ANNUAL MEMBERSHIPS PAID ONE YEAR IN ADVANCE.

Individual membership is $11.95  per month

and family membership is $19.95 per month.

Family membership includes all residents in the household, including children, parents, relatives, significant others, and all permanent residents of the household.

Q. How much more
do the pharmacy and Vision memberships cost?

A. The Prescription Drug and
Vision Care benefits are absolutely FREE with the Dental Plan membership.

Q. Who are the Dental,
Pharmacy, and Vision Care providers?

A. We believe we have the largest
provider base of any discount fee for service dental plan and are continually adding new dentists to the program. These dentists are well-established with successful dental practices. The Prescription Card may be used at over 35,000 pharmacies nationwide. The Vision Care benefits are available at over 12,000 providers nationwide.

Q. How do I use the Plan?

A. Within 10-14 working days
of your enrollment, you will receive a Member Information Guide, Membership Identification Cards, and a Dental, Pharmacy, and Vision Care Directory.
Present your Identification Card to any provider and you will be eligible to get the discounted fees. You pay the provider at the time of service.
