88-1689 | 18 January 1991 | 336 TFS | 'MUDHEN 56' was destroyed on mission to bomb the Al Basrah powerplant during operation Desert Storm. Cause: It's unknown exactly what happened to this aircraft. None of the other flight members on the mission saw any explosions which could account for the crash. It is believed that they became disoriented during a AAA or SAM threat reaction and collided with the ground. Both crewmembers were killed and apparently did not attempt to eject. |

89-0504 | 18 April 1995 | 336 FS | Crashed into the Atlantic off the coast of North Carolina during a night 2v2 air-to-air training mission. The pilot ejected at 780 KCAS (over Mach 1) and was subsequently rescued; the WSO died from injuries sustained in the high speed ejection. Cause: A HUD failure caused the pilot to get into a nose-low, high speed dive. |

89-0497 | 21 October 1998 | 391 FS | 'MILLER 02' Crashed during a night training mission 80 miles south-west of Mountain Home AFB in the Paradise MOA northeast of McDermott, Oregon. Both crew members were killed. Cause: Night 'spatial disorientation' by the crew caused them to unknowingly fly the aircraft to a nose low position outside terrain following radar (TFR) limits. By the time the "low altitude" voice warning sounded, it was too late to recover from the steep dive. |
