On this 19th day of December 1881 personally appeared before me, a clerk
of the circuit court, a court of ??? within and for the county and State
aforesaid, Jason B. Mullinnex aged fifty four years, a resident of the
county of Jackson and state of West Virginia, who being duly sworn according
to law declares that he is the identical person who was enrolled on the
22nd day of August 1862, in Company K of the 11th Regt. of West Va infantry
commanded by Captain Newton Poling and was honorably discharged at Richmond,
Virginia on the 17th day of June 1865.

That while he was a member of the organization aforesaid in the service
and in the line of his duty, on the Doublin and Lynchburg raids in the
Spring of the year 1864, he contracted piles, rheumatism, and nervous prostration,
which greatly disabled him at the time, and has so disabled him ever since,
that he was not treated in any hospital for said diseases, that he has
not been employed in the military service otherwise that stated above,
that since leaving the service this applicant has resided in the county
of Jackson in the State of West Virginia and his occupation has been that
of a farmer, that prior to his entry into the services above named he was
a man of good sound physical health being when enrolled a farmer that he
is now partially disabled from obtaining his subsistance by manual labor
by reason of his injuries above described received in the service of the
United States, and he therefore makes this declaration for the purpose
of being placed on the invalid pension roll of the United States.

That he has never received a pension, that he made application for a pension,
which is numbered 431,559, and in which claim this is intended as an amended
