On this 19th day of December 1881 personally
appeared before me, a clerk
of the circuit court, a court of ??? within
and for the county and State

of the
county of Jackson and state of West Virginia,

11th Regt. of West Va infantry
commanded by Captain Newton Poling and was
honorably discharged at Richmond,
Virginia on the 17th day of June 1865.
That while he was a member of the organization
aforesaid in the service
and in the line of his duty, on the Doublin
and Lynchburg raids in the
Spring of the year 1864, he contracted piles,
rheumatism, and nervous prostration,
which greatly disabled him at the time, and
has so disabled him ever since,

said diseases, that he has
not been employed in the military service
otherwise that stated above,
that since leaving the service this applicant
has resided in the county
of Jackson in the State of West Virginia and
his occupation has been that
of a farmer, that prior to his entry into
the services above named he was
a man of good sound physical health being
when enrolled a farmer that he
is now partially disabled from obtaining his
subsistance by manual labor
by reason of his injuries above described
received in the service of the
United States, and he therefore makes this
declaration for the purpose
of being placed on the invalid pension roll
of the United States.
That he has never received a pension, that
he made application for a pension,
