2) Go>Edit>Copy>Edit>Paste as a New Image. Set aside.

We're going to place 4 spots on your image like this:
Let's take the Easy Route... :)
5) Add a New Layer. Deactivate Layer 1. On your New Layer (Layer 2) Place your First Dot at 50,50. Right Click on the Dot Layer Layer>Duplicate>Image>Mirror.... Merge the 2 Dot layers. Once Merged...you should only see 2 Dots.
Right Click on the merged layer>Duplicate>Image>Flip. All 4 dots are in place now. Reactivate Layer 1. Merge All the Layers. Merge>Merge Visible.
6) Go>Effects>Texture Effects>Mosaic Antique and apply these settings:
7) Now Go>Effects>Texture Effects>Mosaic Glass and apply this settings:
8) Remember that New Image that was set to the side? Click on the Image>Edit>Copy.
9) Go Back to the Template Image. Paste as a New Layer or ADD a New Layer and Paste as a New Selection... whichever you prefer.
Position the image on any side. Add a Drop Shadow. On the other side you can add your own Text or the one I have provided on the zip file. The same one Shown below on the Right. Just Open>Edit>Copy>Paste as a New Layer or Add a New Layer and Paste as a New Selection. Position.
All Done ! :)
You can frame your Image or leave like it is. I added a border of 5. Flood Filled with the pink color. Then I applied Bladepro with the soclear preset.
I hope you enjoyed the Tutorial. :)
