To server food in the greater Lombard area, you'll need two things.

 

Certificate of Occupancy and Business License.

 

In order to obtain these two, you have to go through serious of inspections.

 

To obtain Certificate of Occupancy, they need approval from the following inspections.

 

1. Health Dept Inspection, 

 

2. Fire Safety Inspection,

 

3. Building Inspection,

 

4. Sign Inspection.

 

Once all of these departments approves, you get your

 

5. Certificate of Occupancy.

 

After you receive your Certificate of Occupancy, you'll receive your

 

6. Business License.

 

You can't by skip any of these steps. This has to be in sequential order.

 

 

 

 

During our first health Dept inspection,

 

 

1. Repair Self Closer to Back Door

                Self-closer which is like spring…you need to replace that so it will close automatically

 

2. Demonstrate that Freezer is reliable. Weather proof etc. Must hold temp at 41 degree or below.

Unit will have to be sealed wall. Provide evaluation report on freezer.

                You need to put the door

                You need to seal well to the wall using some sort of foam or coalk.

                Freezer has to be cleaned and serviced and make sure it is working and freeze the food

 

                Need freedoor and test it..

 

                Trim & silicone

 

3. Repair all broken floor tiles

                Tiles are broken near kitchen area. Needs to be replaced

 

4. Provide 25% of square feet for dry storage space.

Need??? (25% of kitchen area or total area) square foot of spaces for storage.Resolve it.            

Where to put shelf or such thing. What kind of diagram. Need to knock/extend the wall.

                               

5. Remove wooden block under water line.

                This is in storage room. Need to find out how to solve the problem.

 

6. Relocate water lines 6 inch off the floor (3-basin sink)

                Make it up the ground

 

7. Have grease trap pumped out

                Service it.

 

8. Remove walls around mop sink

                Both sides you need to knock down the walls around the mop sink

 

9. Provide proper electrical covers.

                Buying and installing it

 

10. Reach in coolers w/ formical tops must be replaced with NSF approved coolers

                There are 2 coolers are meant for bar not in the kitchen. So replace it.

 

11. Remove wire shelving on walls in kitchen and all brackets.

                There 2 places around the kitchen. Replace it with something else. Or remove it

 

12. Gaskets on preptop cooler are in disrepair. Provide new gaskets

 

13. Door hinges on sanchwich preop cooker oare in despair,. Replace

 

                These two are on the cooler.

 

14. Seal voids around pass through window.

                There is space in pick up window. There is a gap. Fill it. Remove the window.

 

15. Contact surface to clean kitchen hood

 

16. Men restroom door is not closing all the way.

 

17. Provide covered waste container in women’s restroom

 

18. Provide towel dispenser at front handsink near the front counter

 

19. Paint and seal front counter to wall,. Provide base coveriong and limit storage to cabinet

Or counter can be removed.

 

 

 

During our Second Health Dept inspection,

 

 

1. Adjust Back Door to close Tightly

 

2. Remove Center shelving unit or provide shorter post for walk-in

 

3. Clean & remove all unnecessary items from facility (dry storage area)

 

4. Clean solid walls

 

5. Install base coving around mop sink- make sure grease trap lid can be removed w/o

 Removing base coving

 

6. Install all shelving above 3-basin sink & mop sink

 

7. Provide 3 stoppers for 3-basin sink

 

8. Provide mop hanger

 

9. Clean floor

 

10. Provide quick disconnect for gas lines along the cook line

 

11. Provide proper filter – no gap on hood

 

12. Gaskets on prep top coolers needs to be replaced

 

13. Adjust self-closers on bathroom door

 

14.Repai                r base coving in men room

 

15.Clean door leading into kitchen

 

16. Evaluate front counter prior to opening

 

17. Install cooler

 

 

During our Third Health Dept inspection, we got approved with one improvement comment

 

1.        sandwich cooler has to hold at or below 40 degrees

 

 

 

 

During our Sign Inspection,

 

 

1. Provide access and installer to verify that installation is code complaint

 

2. No access to xfm area is provided - needed

 

3. Make necessary arrangement to facilitate above

 

4. Properly hang window neon - no contact with metal frame or with blinds.

 

5. Need the installer of the sign to discuss

 

 

During our Second Sing inspection, which was the next day, it was approved

 

 

 

During our Fire Inspection, It was approved with the comments

 

1.        Address of the store has to be on the back door

 

2.        hang the fire extinguisher

 

3.        need the key to the premise
