Voluntary Disclosure of Information
Our
Club has a responsibility to protect our players from potential harm. This
is a concern on the national level, and is discussed under the topic of
Risk
Management. Our Club has a policy that all persons who may act in an
official capacity on behalf of the Club are required to complete a Voluntary
Disclosure Form. This is required of all

Board
members (including consultants
-
Coaches;
head coaches and assistant coaches for all teams
-
Referees
over the age of 18
The voluntary
Disclosure Form seeks information related to certain types of criminal
activity. The form is based on that described at the national level of
soccer. Relevant individuals should obtain a copy of the form, print it
out, sign it and return it to a representative of the Club (e.g., Board
members return it to the Club President; coaches return it to their respective
Coordinator who will forward it to the Club President). You can get a copy
of the form here.
It
is the responsibility of the Club President to maintain original copies
of the signed forms in confidence. The NASC Executive Committee shall
have responsibility for resolution of issues regarding voluntary disclosure
of information.
