Conditions of Registration

Registering Online

This form is to be completed by a parent of the boy applying for registration.

I understand that completing the details in the forms below does not constitute an automatic registration for my son in Melita F.C. Youth Nursery. My son will be considered a registered player only when the payment of the annual registration fee and contribution (see below) is paid, accepted and a receipt is issued.

Previous Nurseries

If my son was registered with another nursery in previous years, I understand that it will be my obligation to obtain a release from his former nursery or nurseries. Melita F.C. Youth Nursery does not pay any compensation to obtain release of your son from his former nursery or nurseries.

Registration and Contribution Fee

The registration fee for Season 2003/2004 is Lm30. An obligatory contribution of Lm40 must also be paid in addition to the registration fee, which contribution will go towards financing Phase 2 of the Melita Nursery Project. I undertake to pay the fee and contribution as soon as I am called upon to do so.

Refusal of an Application

I understand that this application can be refused and Melita F.C. Youth Nursery is not bound to give any reason whatsoever for refusing this application.

Other Registration Requirements

You will need four passport sized photographs of your son together with an original birth certificate if your son is accepted to join our Nursery. Payment of the registration fee and contribution is to be made by cheque but you must effect payment only when you are asked to do so. DO NOT send anything OTHER THAN THIS FORM until your son's application has been processed and accepted, and you are notified accordingly.
