Festival of the Arts University of Chicago Reunion

I will participate in FOTA Gallery during reunion and submit a registration form for each original work or collection of works by May 9, 2003. All works will be accepted for exhibition unless too large in size or in weight or there is an overabundance of submissions for the space. You will receive follow up confirmation. For visual arts works please promptly send, for our “FOTA on the Web”, a photograph--preferably jpeg or gif--to dann@alumni.uchicago.edu, print photographs of submitted work accepted. Poetry if submitted electronically, can be placed on the web. Links to your home pages will be made.

* This feature was enjoyed very much by fellow alums in ’98. Hope you will be able to participate.

TERMS AND CONDITIONS

(1) I certify that the work submitted is my original creation or mine with other persons named on the work, or, if performing the work of another, that the creator is properly identified and that no royalties or commissions are due on account of this performance.

(2) I understand that I must submit the work at least one (1) week prior to the Reunion or bring it on June 5, 2003 for set-up.

(3) I understand that while the University of Chicago will provide security in the building and exhibition area, it is not a guarantor of the safety of the work submitted and that I will bear the full risk of loss at all times. I do have the right to insure the work against loss or damage while at FOTA and in transit solely at my own expense.

(4) I assume full responsibility for shipping the work to and from the University and the FOTA exhibition.

(5 ). This registration is only for exhibiting the work and does not waive any of my rights to ownership, possession, copyright, rights of reproduction, or any other rights in the work or portion of it.

(6) I hereby release the University of Chicago, its Alumni Organization, the Class of ‘63’s 40th Reunion Committee and its volunteers and workers from any and all liability arising out of or relating to the shipping, mounting, display and dismounting of the work(s) submitted by me for FOTA 2003.

(7) I hereby grant full and free license to the Class of 1963 40th Reunion Committee and the University of Chicago to reproduce this work or portions of it in photographic, video, electronic, or printed form to share with University of Chicago alumni and the University of Chicago Library or archives. I also grant license to feature this work or portion of it on the Class of 1963 and Reunion websites and in publicity for this reunion and future reunions of the Class of 1963.
