New Patients If you are a new patient wishing to join the practice you will be asked to complete a registartion card if you do not have a medical card. You will be given an appointment to see the doctor or the nurse for a routine medical examination which is helpful to your new doctor as it usually takes a few weeks for your notes from your previous doctor to arrive. Change Of Name / Address Please inform the receptionist if you change your name or address so that your records can be amended. Results Of Tests Please phone for any results after 12.00 noon.
