The NASA (ASRS) General and Controller Incident Reporting Forms may be downloaded in Adobe Acrobat format. If you do not have the Adobe Acrobat Reader software on your computer, you will need to download a free copy of the Reader directly from Adobe
(http://www.adobe.com/prodindex/acrobat/readstep.html)
for Windows, DOS, Macintosh, or other platforms.

NOTE: The General and Controller Reporting Forms are normally printed by NASA on double-sided, legal-size (8 1/2 x 14) paper, but because many users cannot easily print two-sided, legal-size pages, these electronic forms are provided here as single-sided, letter-size (8 1/2 x 11) forms, thus you will have to print on two pages for the form.

After downloading and printing the form, fill out both pages completely (use extra paper if required), attach both (all) pages together, enclose in an envelope, seal, affix sufficient postage, and mail to:

Please do not fax the completed report.

	Download free Adobe Acrobat Reader software directly from Adobe.
	Pilots, Dispatchers, Airport Personnel: Use the "General" Reporting Form.
	Air Traffic Controllers: Use the "ATC" Reporting Form only.
	Mechanics: Use the "Maintenance" Reporting Form only.
	Cabin Crew: Use the "Cabin Crew" Reporting Form only.
