The Stanza on Merlin's Grave
Bedd Ann ap lleian ymnewais fynydd
Iluagor Llew Ymrais
Prif ddewin merddin Embrais.
Translation:
The grave of the nun's son on Newais Mountain:
Lord of Battle, Llew Embrais,
Chief Magician, Myrddin Emrys
I have only ever seen this in the spurious 21 Lessons of Merlyn (a rather dangerous hoax in terms of historical accuracy, druidic philosophy, and "herbalism" which suggests the ingestion of toxic chemicals). However, I'm willing to guess that it's origin is in the Myvyrian Archaiology, which is also the origin of the epitaph for Mabon ap Modron. Now, the question is then whether these two epitaphs are, in fact, authentic medieval relics, or are they the work of Iolo Morgannwg's fertile imagination.
