The following information should appear at the head of every document generated during genealogical research.  It will help keep track of what you have found, where and when you have found it.  Always keep in mind what all the genealogists stress:  memory is fallible; detailed record keeping is an absolutely necessity!

If the source is an individual where and when is he/she being interviewed:

If the source is an item (e.g. tombstone), what is its location:

If the source is a record, what is the repository in which it is located, what is its location within the repository, when was research undertaken:
