Narrative Based Medicine Questionnaire

It may be useful to ask patients to fill out a narrative based questionnaire in which they write about their illness or problems. We have provided a downloadable rich text format file of questions that elicit narrative information for physicians to use or modify. The questionnaire may be used in the course of the doctor-patient interview or can be given to the patient beforehand in preparation for the clinical encounter.

These questions are structured to elicit the basic elements of a narrative which, in turn, may allow patients to reconstruct a mini-drama. The result may help situate and contextualize an illness, symptom, or other medical concern in the day-to-day life of the patient, thus assisting the physician in building a narrative picture.
